Thoát vị túi mật qua khe Winslow là một tình trạng hiếm gặp, thoát vị kèm xoắn túi mật qua khe Winslow lại càng ít gặp hơn. Bệnh cảnh lâm sàng phức tạp và hiếm gặp tạo ra cho các nhà lâm sàng nhiều thách thức trong chẩn đoán xác định tiền phẫu lẫn thái độ xử trí. Chúng tôi báo cáo một trường hợp nam giới 70 tuổi, nhập viện với triệu chứng đau bụng vùng hạ sườn phải, thượng vị kèm sốt. Hình ảnh cắt lớp vi tính gợi ý thoát vị túi mật qua khe Winslow, chúng tôi tiến hành mổ nội soi cắt túi mật và phát hiện cuống túi mật bị xoắn gây hoại tử túi mật, bệnh nhân sau đó được chuyển mổ mở cắt túi mật. Hậu phậu ổn định ra viện sau 10 ngày. Thoát vị túi mật qua khe Winslow, đặc biệt kèm theo xoắn túi mật là một tình trạng hiếm gặp nhưng có thể gây ra các biến chứng nguy hiểm. Phát hiện sớm qua chẩn đoán hình ảnh và can thiệp phẫu thuật kịp thời là yếu tố quan trọng giúp giảm thiểu nguy cơ tử vong và các biến chứng nặng.
